Created at the request of Newcombe House Family Practice and the Chilliwack Division of Family Practice. This eForm was created with the Permission of Dr. Margaret Weiss and specific permission was received to post this on OSCARGalaxy.ORG . The Original fillable PDF can be found on the Canadian ADHD Resource Alliance (CADDRA) web page. Posting to other sites requires contacting Dr. Weiss.

This eForm calculates the Mean scores for categories and excludes the NA replies in calculating the Mean. There is also a button at the bottom to identify if a question has been left unanswered (unanswered means no selection including not selecting NA). The unanswered questions will be reported in an alert and those lines will have yellow backgrounds. Once a question has been answered, using the button again reverts the background to the original colour.
