How to import an eform that is in a zip file

Created for Dr. Wood by OSCAR BC - an updated version of the Alternate Island Health Consent Form. This new form converts the date information into a day with a suffix (st, nd, rd, th) a month and a year. It also parses the OSCAR time to give Hours and minutes but not seconds. There is a drop down list for options of witness relation to the person receiving the procedure. There is also a link to the booking form and the regular consent form.
