ICBC CL489A Extended Report eFORM

How to import an eform that is in a zip file

Use this eFORM if the patient is unable to work, etc, after their crash.

Updated with Feb2023 background.  Requires creation of several ICBC-related measurements:

ICCN = ICBC Claim Number, ICVN = ICBC Vendor Number, ICDA = MVA date.  The first two have no validation, and the last is a date.  If unsure how to create these measurements, as your OSP for assistance.

The CL489 Standard Medical Report eFORM is used when the patient is able to work, etc, after their crash.  Found here.
