ICBC CL489 Standard Medical Report eFORM

How to import an eform that is in a zip file

Uses this eFORM for patients who have not missed work, etc, as a result of their crash.

Updated with Feb2023 background.  Requires creation of several ICBC-related measurements:

ICCN = ICBC Claim Number, ICVN = ICBC Vendor Number, ICDA = MVA date.  The first two have no validation, and the last is a date.  If unsure how to create these measurements, as your OSP for assistance.

The CL489A Extended Medical Report eFORM is used when the patient is unable to work, etc, after their crash.  Found here.

The CL489B Reassessment Medical Report eFORM is used when ICBC requests an update on the patient's status after their crash.  Found here.
