How to Import an e-Form
Use download button below, then import eform into your EMR by following these instructions
Created at the request of the Chilliwack Division of Family Practice and the Southgate Medical Clinic. Imaging Request for Work Safe BC. This eform will check if the patient's EDC (or EDD) is anywhere between 34 weeks in the future and 3 weeks in the past and check off pregnant. It also checks for allergies, metformin, and diabetes to a greater or lesser degree of accuracy.
