Special Authority: Generic Form

This is the HUB eFORM for most medications requiring Special Authority approval.  Note that Special Authority also promotes their web portal, also called eFORM!   Their own admission: "An eForm is an online version of a paper form".  So if you enjoy autopopulating most of the eFORM yourself, you reap the benefits of almost instant approval - if that matters.

V36_Jan8_2023 is a compilation of many contributors, most recently Dr. Gunther Klein who provided a unified look to the side-panel, and many others including Dr. Herb Chang, Dr. Paras Mehta, and myself.

V37_Jan10_2023 has the side-panel detuned for less distraction.  Thanks Dr. Klein.

V39_Jun28_2024 has been made "Prettier" and run through the "Validator".  Dead links have been corrected.  Fully functional only if associated SA eFORMS also in your system.

Enjoy it for as long as Pharmacare allows us to use this format.
