Multidisciplinary Obesity Clinic Coquitlam 2024

Created at the request of the Newcombe House Family Practice group. Referral to the Multidisciplinary Obesity Clinic in Coquitlam directed by Dr. Micheal Lyon. The eform is able to convert lb to kg and ft&inches to cm. There are also 2 buttons for calculating BMI (depending on the criteria for referral). Page 2 oddly requests lb, so the conversion is made from the chart value in kg back to lb. There is a check to see if a weight has been previously entered in the chart, and the same for BMI. There is also and age check and a BMI check.
