How to import an eform that is in a zip file

Created collaboratively by Drs. Herbert Chang and John Yap. Updated, V8_Feb12_2024 to include copyfax, and cues to DVT algorithm, triggered when ordering vascular Doppler ultrasound.

V9_Feb13_2024 now has full DOAC pop-up to provide decision-making details to radiologist.

V10_Feb4_2025 fixes DVT protocol bug that did not re-display on reload of eFORM.
