Lookup all patients who have the selected eForm

eForm Where Used List

This will list all the patients who have a specified eForm. It is used to report where an eForm was used (in what patient's chart you saved an eForm).

Updated by Adrian Starzynski 2023.12.20 to include the saved form's ID, the form provider ID and name from provider table (to see who saved the form).

TIP: to quickly navigate to an eForm by fdid, go to "/oscar/eform/efmshowform_data.jsp?fdid=123" where 123 is the fdid.

Updated to link the saved eForm ID to open in a new tab, so you can go directly to the eForm from the report. Also includes link to open demographic directly from the report.
