How to Import an e-Form
Use download button below, then import eform into your EMR by following these instructions
This eFORM captures most of the "private" imaging facilities in the New West/TriCities and nearby areas. The current upgrade includes the recent updated Perinatal Services BC (PSBC) recommendations for first trimester dating ultrasound.
There are links to VMI Langley/Abbotsford/Chilliwack and MedRay eFORMs included in this eFORM.
V31 now includes Tickler loop closer and Oscar Fax capability.
V32b has Tickler link, which is not automated. The OBS calculators are now functional.
V35 has signature stamp/script as WCMI now insists on a signature on their requests.
