Fraser Health Diabetes in Pregnancy referral eForm updated to 2023
Updated 2023/12/01/ 11:59 - the form now has a new background. Code for Faxing, copying fax numbers, pulling pregnancy information, and changing the FP information based on whether the FP is another member of the same office, or if the patient was referred in from another office (you need the referral doctor # filled out in the demographic for that to work).
