Centre for OASIS Referral eForm

How to import an eform that is in a zip file

I was asked by a colleague to create this eForm. Referral to the Centre for OASIS (West Broadway, Vancouver). ( OASIS = Obstetrical Anal Sphincter InjurieS - 3rd & 4th degree tears) The original PDF is 4 pages, but the first 2 pages are information only. I have put those in the accordion menu on the side. This is now a 2 page eForm (with lots of room).
