How to Import an e-Form
Use download button below, then import eform into your EMR by following these instructions
This eFORM does not save to the chart. Rather, it pulls necessary data to allow for the risk engine - Framingham by default - to calculate the baseline risk. Choose treatments to see the effect on risk. This is a modification of the previous CVD risk calculator from Dr. J McCormack et al.
Print the results to PDF to share with patients, or upload to the patient chart. OR use the EMR note which you can save to your clipboard and paste to the eCHART.
