How to import an eform that is in a zip file

Created on request to OSCAR BC, by Dr. John Yap.

The target for this eFORM is a bit confusing.  It references Cranbrook as the population location, an ordering physician located in Vernon, and targets patients who may not have a health care provider.  It allows the patient to self-select certain tests.  There is the option for the MRP to receive copy of the results, presumably if the MRP is actually issuing the req.   Note that all these tests can also be requested using the regular Lab requisition.

V2_Jul29_2025 has additional erroneous input field removed. It is otherwise unchanged.
