V12_Apr4_2024 has updated cues for required field. Please add a primary diagnosis. Extra X-boxes printing on page 2 now fixed.
The Plan P form has been simplified, but expanded to a 2-pager. It takes advantage of the signature stamp. Unfortunately, the link to local Home Health fax numbers is less than helpful.
This was previously associated with the many Special Authority forms.
