Special Authority: Dapagliflozin for Heart Failure with Reduced Ejection Fraction (HFrEF) eFORM

Dapagliflozin (aka Forxiga) now covered for heart failure when used in conjunction with other additional HF meds - ACEI/ARB, beta-blocker, and Aldosterone antagonist.  Note that this drug is also used in Diabetes.
