Special Authority: Asthma Triple Inhaler Therapy

This specialized eFORM is only for asthma patients who need the most aggressive inhaler therapy - namely triple therapy. Note that SA is already required for ICS-LABA combos, but those request can be made on the generic eFORM.

Two other tripleRx inhaler products exist, but are approved for COPD only.
