Finally - signature stamp enabled. Note that HIBC will not accept this form without ALL signatures complete, especially the patient consent on page 1.  Unless the patient is new to the province, without MSP, it is not easy to fulfill the criteria for a verbal consent, if the patient is unable to physically sign the form.
