Created on request by IHA. This form is clearly directed at the patient and medical imaging, more than the Family Physician. Page 2 is for the the imaging tech/radiologist to complete, so it is simply the "image". There may some issues not considered by IHA when they created this form - such as how they will contact the MRP!
