Interior Health Authority Standard Outpatient Breast Imaging Requisition

This is the "generic" eFORM for all sites providing breast imaging services in IHA.  This version is not fax-enabled, unfortunately.

V3_Mar14_2024 corrects error in RIH site location.  Note there is a new Rae Fawcett Breast Health Clinic also in Kamloops which is a "one-stop" option for any clinical high risk breast lesion.
