If you are having issues with fax disclaimers appearing in the printout of these eFORMS, upgrade to the Apr10_2023 versions, which introduces a blank background to push the disclaimer off the main page.  Unfortunately, this may create a 2nd blank page, but at least the eFORM will be legible.  No need to change if your current version is working.
