Fraser Health Pulmonary Diagnostics Referral 2022
Created at the request of the Division of Family Practice in Chilliwack. Referral to Pulmonary Diagnostics in Fraser Health. There is redundancy in the eForm to allow for multiple work flow options. The patient handout can be printed from a link or by using one of the print buttons at the bottom of the page. The fax numbers can be copied when clicked and the ability to fax from OSCAR is also coded in. Specific portions of the second page are also available in the side menu.
V18_Mar2_2024 incorporates the latest Oct2023 background, and is an update of an earlier version of Dr. Robertson's eFORM. Note that Drop-In spirometry is currently NOT AVAILABLE at RCH. At some point, the side-menu will be added to this version too. CopyFax functions added. Other than that, there are mostly cosmetic changes.
As promised, V19_Mar3_2024 incorporates Dr. Robertson's side-menu (minor cosmetic changes and some proof-reading), and the ability to draw on page 2, if desired. This is set to always print 2 pages. Note that RCH does not currently offer drop-in spirometry, so that detail has had a "white-out" treatment. This may change in the future.
