FHA Request to Change Designated Family Doctor or Nurse Practitioner

This form is specific to FHA and is designed to notify the Health Authority of a change in primary provider for patients who have been previously admitted to a FH hospital.  The intent is to ensure continuity of care as patients transition from one provider to another.
