V4_Jun22_2022 has stamp and wet signatures enabled, with the stamp set as default The original version was designed to accommodate CGH iron infusion, and included RCH, JPOCSC, and CGH sites. Note that not all sites can accommodate all requests. Also note that there seems to be a specific Venofer infusion eFORM. This version has been upgraded with a phlebotomy option with suggested orders. The CGH infusion form had a formula to calculate estimated iron restoration needs, but I could not confirm any of the calculations, and have left the function commented out. The reference PNG is still in the zip file. JohnY
