Created by Khai Phan: The new Burnaby central COVID-19 site opened on March 28. The site operates by referral only. Please DO NOT send patients directly to this site. This form is only for use by Burnaby FPs/NPs or FPs/NPs that have patients residing in Burnaby: 1. To refer a patient for testing who have moderate symptoms and that you have deemed to be at risk of hospitalization. 2. To refer patients suspected of being exposed to or having contracted COVID-19 who need an in-person primary care visit following a virtual or telephone visit with you.
