At Home HPV Requisition

In BC, the Cervical Cancer Screening Lab (CCSL) has rolled out its Cervix Self-Screening program, issuing HPV swabs (PCR for HPV DNA) to patients directly.  Some sites are doing these tests in office or providing the kits to their patients who attend for scheduled Paps.  For those patients, you can use the paper "At Home" form provided, or this eFORM.

The key is to put the collection date on both the req and the specimen tube.
