FHA Complex Wound Care Referral Form

The complex wound centre (CWC), located in Port Moody, specializes in the comprehensive treatment of lower limb wounds, and can also assist in the treatment of non-healing post-surgical wounds.  The phone numbers are updated in V2_Nov5_2021.

Please ensure the patient meets the referral criteria:

Patient will need to be ambulatory

Patient does not have a sacral or coccygeal pressure wound

Patient is able to transfer independently from a wheelchair.
