Diabetes (DM) Advisor

Created by Dr. John Yap, based on Diabetes Canada template.  This is an alternative to the built-in DM flowsheet that exists in Oscar.  It has space for 3 unique visits. Each time the eFORM is opened, it pulls results from the previous visit and puts it into encounter #2 or #3 as appropriate.  It is meant to work like a flow sheet.  The links are all circa 2021, and hopefully are still active.
