Requisition for Sleep Disorder Consult (Form B) Referral Form
This eFORM was originally paired with the HSAT (home sleep apnea testing) eFORMS which essentially removed all the advertising from the various respiratory support companies and genericized their referral forms. If your patient does not qualify for HSAT, then you are to refer for formal sleep consultation or polysomnogram (PSG), using Form B. Some sites have their own specific PSG forms. The attached are ones using the College's approved format.
The format is similar to the HSAT forms.
