Updated versions of the Complex Care provided by John Yap: I further tweaked the original CC form from Dr. Page to reflect the new ACP tools (MyVoice, MOST). If you have the original CC eForm, then all the handouts should still work. It has new links to MOST, etc. JohnY October2018 updates - CCP Lite and CCP Full are now the new names for these two forms. This allows for easier search for these two related eForms in the search window. There has been some major upgrades to CCP Lite; and only minor upgrade to CCP Full (to allow for the name changes).
This is a very LARGE eForm because of the many handouts. Originally created by the Crossroads Clinic in Chilliwack - many thanks. The update is mainly to accommodate the name change to CCP Full, which allows for easier search in the search box, and ability to distinguish it more easily from CCP Lite. There were no other changes (other than name and CCP Lite link) to this eForm.
