Adverse Events Following Immunization (AEFI) – COVID 19 specific

This is a trimmed down version of the CDC AEFI form that is specifically for COVID 19 vaccinations. It was developed in a FireFox, Mac environment, running Oscar15. Comments welcome. V3 has enhanced phone contact choice function, otherwise is similar to V2.
