As commissioned by the Chilliwack Division of Family Practice this is the updated CGH Cardiology eForm for 2018. Originally this printed on 2 pages, it has been updated to print on 1 page. There is a link on the top left hand corner that will bring up the patient information sheet. This can be printed by using control-P. JohnR.
