Controlled substance prescribing eForm

From PHS Health Care, PHS Community Services Society :

We just completed a major revision of an eForm borrowed from the BC Centre for Substance Use, and want to share it with the OSCAR community in BC. It saves so much hand-writing!

This eForm is designed to print directly onto individual Folio pages carefully removed from the controlled substance pads printed by the College. These pages are then fed into our printers by hand.

Revised for us by WELL, it works with OSCAR Classic and OSCAR Professional. We have named it 1.c OUD eform in our system so it appears near the top of our eForm list. Other users may name it whatever works for them.
