I tweaked the CSS print media script to render a one page paper questionnaire for patients to fill out on a clipboard.
Rearranged the questionnaire to make it less confusing for patients to understand (we are asking about "difficulty" but points are allocated per "level of function")
v2 push-pulls "LEFS" from measurements, and has embedded measurement setup instructions.
v3 has measurements disabled (commented out) for those that don't use measurements
