Created for Dr. Wood by OSCAR BC – an updated version of the Island Health Consent Form. This new form converts the date information into a day with a suffix (st, nd, rd, th) a month and a year. It also parses the OSCAR time to give Hours and minutes but not seconds. There is a drop down list for options of witness relation to the person receiving the procedure. There is also a link to the booking form.

In the process of updating the eForm, all the more recent code improvements were added – including an accordion side menu, Required fields, parsing the date to the correct format, and the latest Xbox and Radio box code.
