<p>This form was generated in the course of the &#8220;Surgical Patient Optimization Collaborative&#8221; (SPOC) project currently being piloted in Campbell River with some DoBC / PSP support.  It is expected that other health authorities will soon have their very own versions of SPOC forms. This is in the context of the current enthusiasm around perioperative medicine and &#8220;ERAS&#8221; principles. The idea is that FPs will work with their patients to address a series of specific concerns brought up by the surgeons/anesthetists to &#8220;optimize&#8221; them in the weeks and months while they are wait-listed for surgery. Concerns such as glycemic control, obesity, OSA, substance use or smoking etc etc.  FPs then are required to report back to the preoperative team in a specific format, hence this &#8220;SPOC&#8221; eForm. </p>
