Created for Dr. Wood by OSCAR BC – an updated version of the Campbell River Surgical Booking Form. This new eForm is compliant with the latest coding restrictions. This form uses Peter Hutten-Czapski’s code to find the date the patient was added to OSCAR as a surrogate for the date of referral. It also circles some inputs. As there is a portion of the form that is repeated multiple times, a javascript was written (with the help of Chat GPT) to copy the first instance of many inputs and paste it to the second and third instance of the same inputs (button activated). There is also a drop down list.

This new form converts the date information into a day with a suffix (st, nd, rd, th) a month and a year. It also parses the OSCAR time to give Hours and minutes but not seconds. There is a drop down list for options of witness relation to the person receiving the procedure. There is also a link to the booking form and the regular consent form.

I was asked by a colleague to create this eForm. Referral to the Centre for OASIS (West Broadway, Vancouver). ( OASIS = Obstetrical Anal Sphincter InjurieS – 3rd & 4th degree tears) The original PDF is 4 pages, but the first 2 pages are information only. I have put those in the accordion menu on the side. This is now a 2 page eForm (with lots of room).

FHA_Consent_for_Care_GP_version_V3_May22_2023 is an updated version of the original, detuned as GP’s may not need the extensive OR forms that surgeons use routinely.
