										<content:encoded><![CDATA[<p>This Grease Monkey script will add English to metric conversion (on double click) and<br />
The script is designed to work in OSCAR  12, 15, 19, OSCAR Pro, Open O, Avaros, and  Juno but is not tested in all of them<br />
In OSCAR 19 and Open-O  automatic BMI calculation is already present, this just adds the English to Metric conversions<br />
The script is designed to work in any Measurement group that has one or more of Height and Weight.<br />
(Obviously both need to be present to auto calculate BMI)</p>
<p>Installation Directions<br />
1. add Grease Monkey extension to Firefox<br />
2. Copy and paste this script into a new GM script window</p>
<p>End User Instructions<br />
Minimal end user prompting is suggested by light blue background for the affected inputs with rollover prompts<br />
it also confirms that the script has loaded!<br />
1. For height conversion you need to enter in the format 6&#8217;2&#8243; (or 6&#8217;2 if you are in a hurry) and double click<br />
2. For weight conversion you need to enter the weight in lbs without any units and double click<br />
3. BMI is automatically calculated once there are entries in both Height and Weight<br />

<p>First: upload the main html to eForms and the images and the css file to images<br />
Second: Setting your default Country. From EVERY computer that will use this tool, click on &#8220;Manual Frax Entry&#8221; (right side at top of this page) which will take you to the Sheffield University Frax Calculator. Hover over Calculation Tool on the top line and select Canada. Do a test run (you only need to enter age, sex, height and weight and then Calculate). Close out the page.<br />
You can now use this eForm as is, click on &#8220;Manual Frax Entry&#8221;, enter the details and then insert the results into the boxes and &#8220;Enter&#8221;. However you can install a &#8220;Calculator Monkey&#8221; to do the work for you, but will involve some customisation&#8230;.<br />
Third (optional): Customising your &#8220;Calculator Monkey&#8221;. Open the greasemonkey script &#8220;FraxCalculator.user.js&#8221; in a text editor. Look for &#8220;https://XXX.XX.XXX.XX:XXXXX/oscar_crossroads/eform/&#8221; and customize this first bit only to your corresponding section of URl that you see with this eForm open. You will need TWO customizations- one for remote access use and one for in office use as the URLs are different.<br />
You are now done! If you went with the Calculator Monkey, it needs to be on EVERY computer that uses this form. All you do is open the eForm, complete any missing information and then click &#8220;Calculate&#8221;. </p>

Alternatively, platelet value can be recorded manually.</p>

<p>Interpretation:</p>

Intermediate FIB-4: Values between 1.45 and 3.25 (inconclusive)</p>

<p>2020-Nov-23 14:23 New DHTMLJavascriptCalender.js file to allow for calendar dates up to 2030. Delete the previous DHTMLJavascriptCalender.js from the image folder and replace it with the new version.  There is also an added js file for CRL based on BCWH tables to calculate EGA from CRL.  From David Page and Peter HC &#8212; SOGC has new guidelines for determining the EDD (Feb 2014): http://sogc.org/guidelines/determination-gestational-age-ultrasound/ the existing pregnancy calculators have been modified for this change. For interest I have added a button &#8220;Previous guideline&#8221; to show what the previous EDD choice would have been. The calculators have several new dependancies: the date calender, the date calculator (I hived these off from the original code to make it more compact), jquery (I think I have chosen the OSCAR version so you likely have this already). The calculators will export EDD to both EDD and EDC so should work in all environments. For BC users there are two new Measurements that need to be created: see the appendix measurement list attached- it is the last two LMPC and USC. These will look ugly in the measurements as they are composite numbers which enable the autoLab to bring in the results to AR 2013. The calculator in AR 2013 is outdated with this new SOGC guideline and I could not export measurements out of AR 2013 so this will be a reasonable workaround. </p>
