Fraser Health North West specific form and generic form for BC. The FNW specific form is customized for Fraser Northwest members – with links to Substitute Decision Maker eFORM designed with assistance from Dr. Charlie Chen, Notification of Expected Home Death, MOST, and ACP Record FHA version. Corrected signature save function when some, but not all signatures are completed on eFORM.

The Generic version works best with addition of Notification of Expected Home Death eFORM.

V4_Jul3_2022 is FHA’s attempt to combine the residential and acute care form into one. Assessment can be done by the MD (for acute care or LTC), or by the nursing staff at residential care facilities. June 2022 background. Signature stamp now included.

Created at the request of the Chilliwack Division of Family Practice. Updated January 9th 2021 to include signatures stamps and cues on Rx dosages.

Just a reminder. For the signature stamp to work you need to store an image (png) of your signature after the style:

consult_sig_XXXX.png

where XXXX is your OSCAR user number. To check for your user number go to administration and click on user management. Then click on search/edit/delete provider records. Put in your last name and click search. In the column ID beside your name (first column on my install) is your user number. Make an image of your signature and save it as consult_sig_XXXX.png where you replace the XXXX with your user number. Save it in the image folder. JohnR.

V23 2025/03/22/ 15:50 Corrected formatting

Updated to include a fax cover page for the Fraser Health Authority ACP repository. Has links to patient info/hand out that can be printed. Also has links to the following forms: CPR Advisor, DNR/No CPR form, and the ACP record. These must be loaded on to your server without altering the names for the links to work. It also has hyperlinks to the Government web page for Representation Agreements and section 7 and section 9. Version 11 now has signature script. Created by JohnY.

V18_Jun29_2022 upgraded with signature stamp, more intuitive data retention for non-patient decision makers, and minor cosmetic improvements. Fixed Print ALL button that was buggy after an initial Print MOST action. Added CopyFax to MOST page. This is still FHA specific, with Cover Sheet to FHA repository. If you are not using the Cover Page, add a blank space to the “pages sent” field.

V19_Aug2_2023 – no longer requires a fax cover sheet. The background was updated Apr2023. The designations are different insofar as DNR is rebadged DNACPR. Overall, it is simpler.

V20 2023/08/06/ 15:49 – Xbox code updated to latest version, Signature script updated to latest version, updated to jquery 2.2.1 with checksum, updated to reImg(), Colours updated to FHA colours, CSS for Xboxes trimmed to remove duplicate code, Circles for section 1 to match the original form, updated parsedate to the latest version, added faxing code, added pdf printing code, added Healthy Surgical Patient button. ***NB*** this eForm has the name “MOST_Elective_Surgery” not “MOST” – to get this form to link with your other forms delete your old eForm named “MOST” and rename this one “MOST”.

As all surgeries large and small in Fraser Health are required to have a MOST form, I have made a simpler version of the MOST form specifically for this. It has a Healthy Surgical Patient button that clicks the full resuscitation buttons.

Complex Care BC 2018 Full Version

Updated versions of the Complex Care provided by John Yap: I further tweaked the original CC form from Dr. Page to reflect the new ACP tools (MyVoice, MOST). If you have the original CC eForm, then all the handouts should still work. It has new links to MOST, etc. JohnY October2018 updates – CCP Lite and CCP Full are now the new names for these two forms. This allows for easier search for these two related eForms in the search window. There has been some major upgrades to CCP Lite; and only minor upgrade to CCP Full (to allow for the name changes).

This is a very LARGE eForm because of the many handouts. Originally created by the Crossroads Clinic in Chilliwack – many thanks. The update is mainly to accommodate the name change to CCP Full, which allows for easier search in the search box, and ability to distinguish it more easily from CCP Lite. There were no other changes (other than name and CCP Lite link) to this eForm.
