This Tampermonkey/GM script originally developed by Braedon Hendy

Updated 2023/01/24 by Adrian Starzynski

It adds a “Copy” button to the patient Master Record when opened from the schedule screen. Upon click this will copy the patient’s demographics (name/HIN/DOB/address) to the clipboard in JSON format:

{“firstname”:”Patient”,”lastname”:”Test”,”year”:”2000″,”hin”:”5555555555″,”vc”:”AB”,”address”:”301 Front St W”,”city”:”Toronto”,”postal”:”M5V2T6″}

Then in the new Healthcare Professional’s Report screen of the WSIB-Telus Provider Portal, there will be a paste option to allow the user to easily paste the demographics.

This script was designed to prevent the need for manual copy/pasting of patient demographics when creating a new e-submission WSIB form in the portal, so providers can more easily submit WSIB forms/reports electronically (which are paid approx. $20 more as opposed to manual fax/mail transmission).

Previously providers were using the eForm version of WSIB Form 8/FAF, however that required manual faxing so it paid less. Now providers can more easily complete the online version through the WSIB’s provider portal and get paid the higher amount, without more effort since the demographics can now be all pasted at once.

2020-Jun-19 14:58 Corrected to include the necessary Doctors_Note_V15.js file. Sorry to the early adopters!

Currently on version 15 – updated for COVID-19 – COVID edition. Improved cues to ensure height of textbox adjusts properly when signing, especially on reload.

Original description:
The original version of this eForm was, I believe, from GuntherK, and was a huge improvement over the previous “sick note” eForm I was using. It is a great platform for using boiler plate – those standard letters for orthotics, CPAP authorization, RTW, etc, etc. The novice “programmer” can add more menu items by using the pattern set out in the js file stored in the image library (Doctors_Note_V8.js). If you’re not sure how to do so, ask your OSP. The original may have had “hard coding” for a specific clinic. I believe the current (attached) one is more “generic” and will reflect whoever is the user at the time. Which means – if your MOA is creating the note, it will be her/his name on the form! The latest upgrade has the fontSizechanger added – because my presbyopia challenges me every day! If there is other boilerplate you would like added, I’d be pleased to help out. If you already have an older version of the Doctor’s Note – you can keep that by renaming it. And you can add this one as a new eForm – with a different name, of course. Then decide which you prefer. JohnY

V19_Feb15_2022 has been further updated to include COVID infection attestation – for those patients requesting exemption from pretravel testing because of recent recovery from actual infection.

V22_Jan18_2023 cosmetic changes, addition of CPAP trial Rx (adjunct to HSAT eFORM), and new comment line below signature. Otherwise, unchanged.

https://github.com/DrStanMD/Oscar-GM4-scripts/blob/master/EncounterNoteEmailerV15.user.js

Darius Opensource has kindly updated the Encounter Note Emailer and Texting greasemonkey script so it is now compatible with Firfox Quantum and Greasemonkey 4+. The file is saved as a text file and must be manipulated for use in Greasemonkey. Additionally – for OSCAR 15, the emailtextengine.js file must be replaced with a newer version. Delete the old version and upload this version. It too has been saved as a text file and the .txt must be removed from the name after downloading and before uploading to the image library.
