I was asked by a colleague to create this eForm. Referral to the Centre for OASIS (West Broadway, Vancouver). ( OASIS = Obstetrical Anal Sphincter InjurieS – 3rd & 4th degree tears) The original PDF is 4 pages, but the first 2 pages are information only. I have put those in the accordion menu on the side. This is now a 2 page eForm (with lots of room).

instructions for adding measurements:

On the main screen click on administration and scroll down to Oscar Encounter and click on Customize oscarMeasurements. Click on add a measurement type. Add the following measurement types, with the first box filled out exactly as below (not inculding the parentheticals), the subsequent 3 boxes can be individualized, and the last box should have either “no validation” for the text input measurements, or the appropriate numeric range.
