It is a lengthy eFORM that requires a wet signature, applicant (aka patient) input, medical (clinician) input, and an assessor component.  The assessor may be the physician too.  Multiple areas require wet signatures, so there is no stamp feature for this eFORM.

There is an MSP fee code for physicians completing this form. 96501 – for Section 2, physician report; and 96502 – for Section 3, assessor report.

Updated V3_Dec26_2022 due to 2021 changes in eligibility criteria – type-1 DM automatically qualify under “Life Sustaining” criteria, and expanded items for Mental Functions.  Web link reference in the eFORM.  Otherwise, essentially the same!
