This has been corrected in the new zip file. The new zip file is labelled Version 2 so that it will import without difficulty. Please delete the previous version to avoid confusion.

2025/05/17/ 14:05 – The new version of this form is now 2 pages long, with the second page being largely blank. The calculators are no longer required. For the required radio boxes I have added a javascript that initially makes all the fields in a group required until 1 of them is chosen, after this the remaining fields are no longer required. Patients under 19 are not accepted, patients between 19 and 25 are considered urgent.

I was asked by The Chilliwack Division of Family Practice and the Newcombe House Family Practice to update this form. It has gone from 1 page to 4. It now has several calculators as well.
