Pathology requisition for Campbell River updated to include wet signature, calendars.

Created for Dr. Wood by OSCAR BC – an updated version of the Campbell River Surgical Booking Form. This new eForm is compliant with the latest coding restrictions. This form uses Peter Hutten-Czapski’s code to find the date the patient was added to OSCAR as a surrogate for the date of referral. It also circles some inputs. As there is a portion of the form that is repeated multiple times, a javascript was written (with the help of Chat GPT) to copy the first instance of many inputs and paste it to the second and third instance of the same inputs (button activated). There is also a drop down list.
