Thanks to Kat Montgomery for alerting me to the re-branding of the EMMA clinic – now the Placental Health or PRISM clinic. There is a link in the PDF that is faulty. Do not click on the MFM OBIM Provincial Guideline link in the PDF. This eForm has the proper link to the recommendations for ASA use. This eform will also calculate the gestational age at the time of referral. It will pull the standard “G” for gravida and “P” for para measurements and determine if the patient is nulliparous. It also checks the maternal age and BMI.

Update 2025/08/28/ 16:07 – the previous zip file did not contain the images because the names of the images had an apostrophe in them. This has been corrected in the new zip file. The new zip file is labelled Version 2 so that it will import without difficulty. Please delete the previous version to avoid confusion.

2025/05/17/ 14:05 – The new version of this form is now 2 pages long, with the second page being largely blank. The calculators are no longer required. For the required radio boxes I have added a javascript that initially makes all the fields in a group required until 1 of them is chosen, after this the remaining fields are no longer required. Patients under 19 are not accepted, patients between 19 and 25 are considered urgent.

I was asked by The Chilliwack Division of Family Practice and the Newcombe House Family Practice to update this form. It has gone from 1 page to 4. It now has several calculators as well.

FIR is a provincial specialized perinatal service located in BC Women’s Hospital and Health Centre in Vancouver BC. FIR provides care to antenatal and postpartum women who use substances, and to infants exposed to substances in a primary care/interdisciplinary team-based model. FIR opened in 2003 and was a first of its kind program in Canada.
